Premier Care Management is looking for Outreach Workers to work with our young people to help develop their values, skills, knowledge, self-control and direction to enable them to live independently.
The role
Premier Care Management provides support and accommodation for vulnerable young people aged 16-25 years. This role is on a 2 days on/4 days off rota basis and will include bank work. The shifts offered can vary in length and cover different times of the day, and could be weekdays, weekends and Bank Holidays.
We're looking for candidates who display good judgement and enthusiasm for working with young people who display very challenging behaviour. You'll also need to be a good team player.
You'll be responsible for:
Working to the requirements of Every Child Matters, Leaving Care Act 2018 and implementing this within the outreach setting
Ensuring that the young people live in a safe environment where their welfare is paramount
Working within PCM's Policies and Procedures and the Company Statement
Engaging effectively with PCM's young people and demonstrate a positive attitude towards them
Developing and guiding our young people to develop the skills needed to live independently and manage a tenancy
Empowering and educating young people to take control of their lives, particularly in training, education and employment
Helping young people manage a budget and pay for utility bills
Helping young people manage their accommodation and support them in keeping the home environment clean. This includes supporting with cleaning duties and gardening
Implementing the care package that has been specifically identified for each young person
Being accountable for time spent with the young person and the actions taken
Completing daily contact notes following each shift completed
Where appropriate liaising with other domiciliary providers in regard to the young person's needs
Participating in delivering, reviewing and monitoring the young person's placement
Promoting anti-discriminatory practices and equal opportunities in accordance with The Equality Act 2010
Attending regular team meetings, in house training and supervisions
The ideal candidate
You'll have:
Excellent communication skills (both written and verbal) in order to record and communicate essential information
Knowledge of basic computer packages such as MS Word and Google docs
An understanding of the demands of working with young people who may have complex needs and the ability to deal with these in a non-judgmental manner
The ability to work flexibly and have the availability to undertake shifts on evenings, weekends and bank holidays
QCF Level 2 or 3 in Health & Social Care (not essential)
Previous experience of working in a similar setting with young people who present with challenging behaviour
Experience of working in a multi-racial environment, or of working with people from other minority ethnic groups
Full UK driving licence and access to appropriate means of travel
